Who am I likely to see during my office visit?

Patients referred to a specialist often require special testing and services not offered in a primary care setting. Providing specialized care requires a comprehensive approach utilizing a highly trained and skilled workforce; depending upon your symptoms, you may see the physician, a Doctor of Audiology, and the nurse practitioner all in one visit. 

What will I need to bring to my appointment?

Primary/secondary insurance information/cards; legal guardianship papers/treatment authorization for minors not accompanied by a parent; Photo ID; applicable co-pays, which are due at the time of service; a current list of medications; test results; CT films/disc (when necessary). Please arrive 15-20 minutes prior to your appointment to complete paperwork if you are a new patient. Patients are seen based on appointment time, or under an emergent situation, not necessarily arrival time. 

What will it cost?

Unfortunately, we cannot determine cost until you have been examined; however, all co-pays are due at the time of service – Visa, MasterCard, Discover Card, personal checks or cash are accepted. The uninsured will be responsible for a payment of $200 for the initial visit. However, this is a partial payment for the office visit with the Physician and/or Nurse Practitioner and Audiologist. The cost of the office visit does not include the use of a scope, hearing tests/services performed by the audiologist, labs or other diagnostic testing required to make a diagnosis. Please ask to speak with our Financial Counselor if you should have any questions regarding cost and billing. 

Will my insurance pay for my office visit?

The disclaimer from health insurance carriers is that they cannot guarantee payment; reimbursement is between you and your insurance carrier. We are very cognizant of the increased financial responsibility the patient may incur and recommend that you contact your insurance carrier directly prior to the visit. In our experience some carriers may have limited benefits for audiology services; please inquire about hearing benefits. We bill your insurance as a courtesy; however, you the patient are ultimately financially responsible. Please be courteous and notify us of a cancellation at least 24 hours in advance so we may use that time slot for others in need. 

Why am I being charged for two co-pays for one office visit?

This charge is between you and your insurance company because you hold a contract with them and we do not. Multiple co-pays are on the rise during the same visit, depending upon the services rendered; the additional co-pay may be billed which will show on your statement. This may apply to audiology services. Always check with your insurance carrier. 

Why do I have a co-pay during a follow-up to surgery?

A postoperative follow-up period may be included in the surgical package (aka: Global period), for services provided related to the original procedure. However, you may be asked to pay a co-pay during this time. For example, you may require a scope that is not part of the follow up care; you experienced ear problems but surgery was for a sinus condition. 

Why do I have to pay to have forms filled out?

The physician is unable to provide patient care when time is spent completing paperwork. There will be a $25.00 charge for some short-term and long-term disability and supplemental insurance forms (life, health, accident). Payment is due when you bring them in or at time of pick up or before being faxed. Please allow 5 - 7 business days for completion of paperwork
